In response to this desire, we have provided a directory to assist you with your arrangements for your arrival:
We are conveniently located next to Central Park in New York City. Our practice is located at: 30 Central Park South, Suite 3B. Central Park South is actually 59th Street, and we are between 5th & 6th Avenues. The office is a couple of buildings west of The Plaza Hotel.
Subway: The N & R lines have exits adjacent to the office.
Parking: Valet parking is available behind our building on 58th Street at the Helmsly Hotel, and the south side of 58th Street; both between 5th & 6th Avenues.
International Travel: With flights into JFK, LaGuardia or Newark Airports car service or taxi can take you directly to the office. For concierge assistance please call the office at +1(212) 832-0808.

In order to expedite your visit please fill out both the Patient Registration Form and sign the Acknowledgement of Privacy Form. It is preferred to print out the two documents and bring them to your first visit, if possible. Printing in black and white (Grayscale) is acceptable:

We always accept new patients into our practice and welcome friends and family. Your referral is the greatest compliment you can give us and it is appreciated. Thank you in advance!
Please give us at least 48 hours’ notice to change or cancel an appointment. If you are scheduled on a Monday, please contact us by the previous Thursday morning. When you are in the office for cleanings, we encourage you to set up your next visit. This will help you to get a preferred time.
If you have dental insurance we will assist you by filing the claim for you. As a rule of thumb if you have a PPO insurance and you can go to any dentist, we will accept it. We are also a Preferred Delta Dentist Provider, but do not participate in any DMO insurances. To find out definitively, please call the office with the insurance information and we will be happy to help you.
